A man should stand up to greet a woman when she enters a room for the first time, or of she arrives at the dinner table after him. 

He should also stand when he is introduced to her, or when greeting her, no matter how informal the situation.

There is no need, however, to be like a jack-in-the-box every time she goes to the loo, goes to get a drink and so on.
